Heritage Tree Program Nomination Form

Nominations may be submitted at any time throughout the year. Any nominations submitted after January 15th meeting the eligibility criteria and approved by the Art's Culture and Heritage Commission will be dedicated the following calendar year.

List the common and Latin name of the tree(s).
The tree must be clearly visible from public view. List the county, street, cross street, or rural route address and direction to the tree.
History and Significance of the Tree(s)
Files must be less than 10 MB.
Allowed file types: gif jpg jpeg png pdf.
Send photographs of the tree(s) and surrounding area. Copies of historic photographs are encouraged. Please date identify the location of tree(s) on the photograph.
